Adkison Family Dental Membership Club Options

Adult Membership(13 and Older) $45.00/mo. (Save at least $134.33 a year!)

Professional dental cleanings (2 per year)
Doctor exams (1-2 per year)
Fluoride treatment
Needed x-rays^*
One emergency exam per year (if needed)
15% off all other treatments (some exclusions may apply)
$135 Lifetime Activation Fee includes first month

For patients with regular cleanings and absence of periodontal disease.

Perio Membership(Periodontal Disease) $72.00/mo. (Save at least $138.33 a year!)

Perio maintenance cleanings (3 to 4 per year)
Doctor exams (1-2 per year)
Fluoride treatment
Needed x-rays^*
One emergency exam per year (if needed)
15% off all other treatments (some exclusions may apply)
$135 Lifetime Activation Fee includes first month

For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.

Child Membership(12 and Younger) $37.00/mo. (Save at least $124.40 a year!)
